Barnardos are looking for a full-time Case Manager for a 6 month contract for the Family Connect and Support Program based in Cranebrook, 2749
The Case Manager will:
- Undertake assessment of the families’ needs and risks utilising the provided assessment framework.
- Provide information, referral and advocacy to support clients to access appropriate services.
- Provide service delivery as per program service specifications and Barnardos Policy, procedure and processes.
- Case co-ordination of clients who may be receiving some interim casework from the FCS but are also being supported to access other suitable services.
- Follow up of clients who are accepted by another service to confirm engagement with the service resources and services in their local areas.
- Provide practical assistance (including financial literacy assistance) to alleviate poverty and disadvantage.
- Monitor child protection issues, consult with their supervisor and act in accordance with the NSW Child and Young Persons (Care and Protection) Act 1998 and Barnardos ‘Child Safety & Wellbeing’ Policy
- Consult and seek guidance from Team Leader for client’s escalating risks and needs.
- Capacity to participate in an on-call roster, providing a 24/7 response to children, young people and families
- Attend regular supervision with the Team Leader.
- Participate in team meetings and where appropriate to attend meetings with the other teams.
- Establish and maintain network relationships with key agencies, other providers, and referral services in all the district areas listed above.
- Work respectfully with clients and provide culturally appropriate support to clients.
- Work collaboratively with the other FCS workers, intake team leader and other team leaders.
About You
As a Case Manager, you will ideally have;
- Tertiary qualification in Social Work, Psychology or Social Welfare related discipline
- Demonstrated experience of working with children and young people and their family in a child protection context, including experience in undertaking comprehensive assessment.
- Demonstrated experience and skills in information provision, intake, assessment and referral
of working with families in need of support services. - Excellent understanding of child protection issues and client vulnerabilities such as drug and alcohol use, domestic violence, mental health issues, and the impact of trauma and the effect on child behaviour and development.
- Be flexible in their working hours and meet the families at a time that is most beneficial to them, e.g. before and after school hours, if required.
- Full unrestricted NSW drivers licence
