Intake and Assessment Officer

Salary: $65,000 - $75,000 per annum

Our Client, a specialised community transport services provider,  is currently recruiting for an experienced Intake and Assessment Officer to join their team based in Inner west of Sydney .

About the role
This rewarding role would require you to carry out intake and assessment of clients from the community with referrals from My Aged Care, NDIS and other portals to help access specialised community transport throughout the Sydney area.
  • Manage new enquires, phone and email
  • Carry out phone-based assessments and reviews of clients using the service
  • Action and manage referrals via MAC portal
  • Record all case notes
  • Manage, invoice and amend and invoice individual accounts
  • Build and maintain relationships with external stake holder
Required skills and experience
  • Certificate is aged care or disabilities
  • 2 years’ experience within the health and welfare or community services sector
  • Knowledge of MAC, CHSP and NDIS portal management
  • Demonstrated ability and experience in dealing with people from culturally diverse backgrounds, older people, people with disabilities and mental health
  • Ability to problem solve, prioritize and manage time efficiently
  • Strong verbal, written and interpersonal communication skills
  • Understanding of Microsoft Office with the ability to manage a variety of software
  • Full working rights in Australia
Desirable but not essential
  • Experience with CALD clients
  • Ability to speak a second language

Additional Information
