Homelessness Case Manager - Aboriginal Identified

Salary: $40 - $42 per hour

Our Client, a leading Homelessness support service,  is currently recruiting for an experienced Homelessness Case Manager to join their team based in Western Sydney.

The role will involve;
  • Deliver on assigned activities and provide outstanding wraparound services and tenancy sustainment to clients
  • Assess and coordinate client referrals at intake and undertake client interviews
  • Develop and support case plans and referrals in collaboration with clients, including timely review and exit planning
  • Connect clients with appropriate support service providers, working in collaboration with all stakeholders
  • Build and maintain relationships with relevant key stakeholders including the local health district
  • Attend and participate in referral and exit assessment group meetings where relevant

They are looking for candidates with a background working with vulnerable people experiencing homelessness 

The requirements for this role include;
  • Aboriginality
  • Qualification in Community Services, Social Work, Psychology or relevant 
  • Work experience in case management 
  • Experience working collaboratively with stakeholders to achieve best outcomes for vulnerable people 
  • WWCC and Drivers License 

This position is Aboriginal identified. To perform this role, it is essential that the person who holds the role be an Aboriginal or Torres Strait Islander person. It is a genuine occupational requirement under section 25 of the Anti-Discrimination Act 1991 that applicants are Aboriginal or Torres Strait Islanders.
