Team Leader - Aged Care

Location: Sydney
Area: Southern Suburbs & Sutherland Shire
Worktype: Part Time
Salary: $40 - $43 per hour

Our client is looking for an experienced Aged care Team Leader supporting their community to get the care and support they need to live a better life's. Being an Aboriginal community focused service provider, they are looking  for a motivated Aboriginal or Torres Strait Islander person, who is a self-starter, with the experience and ability to take on the role of Team Leader supporting clients in the community who are elderly or living with a disability. 

Please note that the position is an identified Aboriginal position. Aboriginality is a genuine qualification authorised under Section 14 (d) of the Anti-Discrimination Act 1977.

The role will involve : 
  • On call support for the team , clients and their carers 
  • Management of Aged Care, disability and Social Support staff
  • performance management reviews, staff movement & training 
  • Strong knowledge and understanding of issues relating to CHSP and DEX program
  • Monitor care delivered to clients        
  • Support team members in aspects of their work ensuring the team works in a cohesive & productive manner 
  • Participate in team meetings
  • Assisting with the implementation of client plans and individual goals 
  • Update and maintenance of client's policies and procedures
  • Support the NDIS program where appropriate
  • Complete all reporting as needed under funding agreements
  • Responsible for Aged Care audit and assisting with NDIS Audit
Selection Criteria:
  • Minimum 3 years’ experience working in Aged Care or disability services
  • Certificate / Diploma in Community Services, Disability Support/ Aged Care or equivalent 
  • Effective communication, interpersonal and negotiation skills, including ability to communicate sensitively and culturally appropriate 
  • Demonstrated ability to work with limited supervision, manage a variety of tasks and display personal drive and integrity 
  • Supervision, teamwork and creative problem solving 
  • Strong leadership skills 
  • A valid driver’s license
  • A current Working with Children Check check and satisfactory completion of a National Police Check
