Registered Nurse - Aged Care

Salary: $36 - $46 per hour

Our client is a not for profit that provides an aged care service providing a long-term home for approximately 60 men around inner Sydney. The residents are mainly older men, who are experiencing homelessness, relying on low pensions and are unable to live independently due to their medical and support needs
  
This service is a home with day-to-day living support for those with a complex medical conditions and limited income. Many residents are living with a mental illness, acquired brain injury or other cognitive disorders, often complicated by a history problematic substance use and trauma associated with homelessness. This service provides a safe environment, socialisation and medical care in a friendly and caring environment
  
This role will involve;
  • Engage consumers using a strengths-based approach to complete a health status measurement and needs assessment.
  • Undertake Nursing duties at a highly proficient level
  • Initiate referrals to appropriate internal and external services
  • Comply with the mechanisms for monitoring, compliance and review of clinical risk management
  • Responsible for ensuring compliance with all WH&S policies and procedures
  • Provide clinical health care services in an aged care facility to those with an alcohol and other drug dependence, mental health issues and/or at risk of or experiencing homelessness
  • Liaise with prescribing medical practitioners and chemists
  • Manage the administration of client medications, to ensure appropriate medical regimes are followed
  • Assess, plan, organise, evaluate and deliver holistic, quality nursing care in accordance with the Australian Nursing and Midwifery Council’s (ANMC) National Competency Standards
  • Document client care accurately and collect and maintain data and electronic health records in prescribed ways
The requirements for this role include;
  • Bachelor of Nursing or equivalent
  • Current registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA)
  • Registered General Nurse with a minimum of 3 years clinical experience in a generalist setting.
  • Demonstrated experience in an effective management of the care needs of HCP
  • A comprehensive clinical knowledge, particularly to age related health issues
  • Strong Commitment to providing excellent clinical care and continued professional development
  • Experience in working with homeless services or community health services. (desirable)
  • Experience in working in Emergency Nursing, the Alcohol and Other Drug sector and/or mental health. (desirable)
  • Appropriately Vaccinated with Booster

Additional Information 
  • A number of probity checks are required including holding a current working with children and undertaking a National police checks.
