Clinical Care Manager

Location: Sydney

Salary: $120,000 - $140,000 per annum

Our client, a dedicatied Aged care provider is recruiting for an experienced aged care professional to become the Care Manager for a new 97-bed residential aged care facility located in Asquith, NSW. The facility is set to open in mid-2023 and will serve a diverse population including, but not limited to, individuals from Korean, Malaysian, Chinese, and other backgrounds. 
This role will involve;

  • Manage the effective and efficient delivery of day-to-day care services, ensuring a high standard throughout the facility in alignment with the Aged Care Act 1997 and best practices in clinical and lifestyle care
  • Ensure the facility is adequately and appropriately equipped to enable quality clinical care
  • Foster a culture of commitment to quality and positivity among management, staff, and consumers, and maintain open lines of communication with families and caregivers
  • Develop and implement care plans for each resident, in consultation with the interdisciplinary team and the resident
  • Monitor and evaluate the quality of care provided and take action to address any deficiencies
  • Coordinate and supervise the work of the care staff, including scheduling, training, and performance evaluations
  • Provide support and guidance to the care staff to help them develop the skills and knowledge needed to provide high-quality care
  • Maintain accurate and up-to-date records of residents' care and progress
  • Collaborate with other healthcare professionals and organizations to coordinate and provide continuity of care for residents
  • Ensure compliance with all relevant laws, regulations, and policies related to aged care
  • Manage the budget and resources of the care facility to ensure efficient and effective operations
  • Act as a liaison between the care facility and residents, families, and other stakeholders, addressing any concerns or complaints in a timely and appropriate manner.
The requirements for this role include;
  • Current AHPRA registration as a registered nurse
  • At least 5 years of experience in a Care Manager role or equivalent
  • Comprehensive understanding of the aged care industry, including regulations and funding
  • Proven track record of compliance and commitment to excellence and quality improvement, with knowledge of Quality of Care Principles and accreditation
  • Experience and knowledge of AN-ACC assessment
  • Familiarity with care planning procedures
  • Skills in people leadership and change management
  • Ability to work effectively in a diverse and multi-disciplinary team
  • Strong interpersonal and communication abilities
  • Proficiency in written and spoken English
  • Adept at using computer and familiar with an IT care system, with willingness to learn new systems
  • Possession of current Australian Federal Police Check.
  • Possession of current Australian Financial Security Authority Check
  • Current COVID-19 3-dose vaccination certificate
  • Current Driver's Licence
  • Desirable: ability to speak and write an Asian language, such as Korean, Chinese (Mandarin or Cantonese), etc.
Additional Information 
  • A number of probity checks are required including holding a current working with children and undertaking a National police checks.
