Care Manager

Salary: $115,000 - $130,000 per annum

Our client is a not for profit that provides an aged care service providing a long-term home for approximately 60 men around inner Sydney. The residents are mainly older men, who are experiencing homelessness, relying on low pensions and are unable to live independently due to their medical and support needs. We are open all year round. They receive funding from the Department of Health and Ageing and NSW Health, with a dedicated team including six registered nurses.
  
This service is a home with day-to-day living support for those with a complex medical conditions and limited income. Many residents are living with a mental illness, acquired brain injury or other cognitive disorders, often complicated by a history problematic substance use and trauma associated with homelessness. This service provides a safe environment, socialisation and medical care in a friendly and caring environment

This role will involve;
  • Leading the organisation in clinical governance in line with the clinical requirements 
  • Working in close collaboration with the DON to meet governance, quality management and client safeguarding requirements
  • Ensure that residents are provided with best-practice evidence-based, physical and mental health nursing care based on thorough professional assessment
  • Ensure the successful implementation of the Strategic Plan within their team
  • Undertaking Clinical Audits
  • Oversight of clinical care plans to ensure compliance 
  • Consolidation of monthly clinical indicators and mandatory reports
  • Adjust resident care plans in consultation with relevant staff and communicating such changed needs as required
  • Collaborate with multidisciplinary teams to achieve desired outcomes for consumers
  • Assess, plan and evaluate individualised consumer care in accordance with company policy and protocols.
  • Provide leadership and support to all nursing and care services staff across and will ensure compliance with the Aged Care Standards and organisational policies and procedures whilst overseeing all clinical responsibilities
  • Ensure key information regarding residents is clearly communicated to care service staff and ensuring this information is readily understood, particularly any relevant changes made to resident care plans.
  
The requirements for this role include;
  • AHPRA registration clinical experience including the ability to recognise and respond to risks in older people with complex and concurrent health needs.
  • Current List A registration with the NSW Registration Board.
  • Clear understanding of the Aged Care Act, as well as Aged Care Accreditation Standards, AN-AAC understanding would be highly desirable
  • Minimum of two years post-qualification experience in Mental Health or Aged Care nursing.
  • Effective communication skills
  • Proven leadership experience and demonstrate ability in management clinical and non-clinical roles.
  • An interest in mental health and aged care
  • Covid Vaccination requirements
Additional Information 
  • A number of probity checks are required including holding a current working with children and undertaking a National police checks.
