Care Manager - Aged Care

Area: South West & M5 Corridor
Worktype: Full Time
Salary: $120,000 - $130,000 per annum

Our Client, a leading Organisation which provides quality care for ageing Australians, is currently recruiting for an experienced Clinical Care Manager to join their team based in South West Sydney, NSW. 
  
This role will involve;

  • Coordinating services to maintain a home-like environment for Customers
  • Actively maintain systems and processes to interpret and capture the choices
    and preferences of individual Customers.
  • Manage relationships with Customers and Families, acting as an escalation
    point for the Care Team.
  • Oversee complaints handling and resolutions – take ownership of responses
  •  Demonstrate clinical leadership in all decision making and actions – assume
    the inherent and explicit responsibility as the Lead Clinician for your Facility
  • Function as a key adviser in relation to clinical care
  • Oversee and coordinate resident assessment and care planning / case
    management processes and documentation.
  • Working collaboratively with the General Manager, monitor and manage the
    financial health of the Facility (revenue; monitor expenditure to budget and
    take corrective action as required).
  • Advise and provide direction to staff on the application and implementation
    of policies, procedures and other information associated with Customer care
    and support.
  • Work with and support the ACFI Coordinator with submission of ACFI uplifts,
    appraisals, and reappraisals.
The requirements for this role include;
  • Registration with the Australian Health Practitioner Registration Agency (APHRA)
  • Tertiary Nursing qualifications (Registered Nurse)
  • Minimum 3 years experience as a Care Manager 
  • Demonstrated Team Leader experience and high level management competence in Leadership; Organisation Management, Time Management, Quality Improvement, Staff Management
  • Experienced within an Aged Care Facility. Working knowledge at a high level in the provision of Aged Care and Disability community programs.
  • Tech Savvy and ability to train others with use of a smart phone, computer and other electronics. 
  • Must be Vaccinated against COVID 19 (including booster)
Additional Information 
  • A number of probity checks are required including holding a current working with children and undertaking a National police checks.
